The direct download link is not static and is generated after login on the official portal. The application form can be reached through Citizen / Family Login, then Applicant Login, after selecting the district and entering the registered mobile number with OTP-based access.
- Visit socialregistry.wb.gov.in.
- Select Citizen / Family Login.
- Choose Applicant Login.
- Select the district.
- Enter the 10-digit registered mobile number.
- Request OTP and log in with the 6-digit OTP.
- Use the Download Application Form button at the bottom right of the dashboard.
The downloaded PDF may include Head of Family details, family members, ration card details, and income and assets information. The form also includes a Declaration & Consent section.
